Supplier Vaccination Requirements

Effective November 1, 2021, MCW will require all contractor and vendor employees providing services onsite at MCW owned or leased facilities to be fully vaccinated against COVID-19. MCW has created an online questionnaire to determine which contractors and vendors must comply with the vaccine mandate. If the questionnaire determines that the vaccine mandate is applicable to your company, you will be directed to sign the MCW Contractor/Vendor Health and Safety Compliance Certification.
